Colorado Massage Company, LLC Policies Agreement

Thank you for agreeing to our standard policies before scheduling and arriving for your first session at Colorado Massage Company, LLC

  1. I understand that failing to arrive for my session or canceling within 24 hours of my scheduled time will result in a charge of 50% of the total price of the session. I understand that unless I have provided specific cancellation payment, the payment will automatically be charged to the card on file.

  2. It is my choice to receive treatment from Colorado Massage Company, LLC.

  3. I am aware of the benefits and risks of massage and give my consent for massage.

  4. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of sessions.

  5. I acknowledge that massage therapy is not a substitute for medical care, medical examination or diagnosis.

  6. I have stated all medical conditions that I am aware of and will inform my practitioner of any changes in my health status.

  7. I understand that my personal health information will be collected.

  8. I understand that all information that I provide will be kept confidential unless required by law.

  9. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment.

  10. Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage, collect receipts from treatment and communicate with my insurance provider.

  11. I understand that sales on Wellness Packages and pre-paid sessions are final. It is my responsibility to track my Wellness Package balance and schedule my package sessions within a three month period, before they expire.

  12. Should I need to arrive late to a session, I will communicate that with my therapist and will be responsible for the time lost unless otherwise agreed upon.

  13. I understand that I am receiving Therapeutic Massage which is non-sexual in nature. Should my therapist find my behavior at all inappropriate, I will be asked to leave without refund of my payment.

  14. I understand that Colorado Massage Company, LLC reserves the right to refuse service to any person for any reason, unless discriminating against a protected class.
